Consultation was provided about the fire extinguisher needing replaced.

An unannounced monitoring inspection was conducted today from 11:00am to 1:00pm with the provider and two assistants. There were eight children in care ranging from 14 months to 4 years of age for a total of 18 points. Five children?s files, two staff files, and two household members files were reviewed. The provider is certified in CPR and First Aid. There were no medications on site today, but the provider has current Medication Administration Training (MAT) certification. Lunch today was salmon or chicken, potatoes, pears, oranges, and milk. The first aid kit, flashlight, weather band radio, fire extinguisher, smoke detectors, and non-medical emergency supplies were reviewed. The emergency drill log and attendance were reviewed and in compliance. Standard #: 22VAC40-111-100-A Description: Based on review of five children's records, the center did not have documentation of a physical in each child's file prior to or within 30 days of first attendance.
Evidence: Child A, with a start date of 8/29/16, did not have documentation of a physical in their file.Plan of Correction: I messaged the parent during the inspection and requested they bring a physical.
