An unannounced monitoring inspection was conducted on October 4, 2019 from approximately 1:20 pm - 2:35 pm . The children were playing with blocks and other manipulatives. Records were reviewed for four children and 2 provider's records reviewed. Upon entrance there were 4 children (8 points) , 1 provider and 1 assistant.

Standard #: 22VAC40-111-100-A Description: Based on documentation and staff interview, the provider did not ensure documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.
Evidence: Child #1 (start date January 22, 2019) did not have documentation of physical examination.Plan of Correction: Provider will contact parent today.
