Discussion was held on the topics of current enrollment (with regards to scheduling and those that have moved on to preschool), and current staffing (with regards to schedule of assistant).
The following inspection took place between the hours of 9:30 am through 11:30 am. Today, I met with the Provider on site at the Family Day Home (FDH) located in Broadlands, Virginia. At the start of this unannounced monitoring inspection there were a total of 7 children observed (6 children in care w/ 1 child resident) (14 points) with 1 caregiver. A total of 6 children?s files, 2 staff files, and 3 household member?s files were reviewed. The children were engaged in the following: free play, toy play, games, outside play, learning activity about nature, bathroom breaks, hand washing, getting ready for lunch, and other organized activities. Standard #: 22VAC40-111-180-A Description: Based on review, 1 of 2 staff files and 1 of 3 household files indicated that the Provider did not obtain for each caregiver and adult household member a current Report of Tuberculosis (TB) Screening form (in accordance with the requirements in 22 VAC 40-111-170), every two years from the date of the most recent documentation on site.
1. On the date of inspection (10/03/2019), the most recent TB documentation on file for Staff #2 was dated - 12/09/2016.
2. The most recent TB documentation on file for Household Member #1 was dated - 03/21/2017.
Plan of Correction: Updated TB documentation for both Staff #2 and Household Member #1 shall be obtained and placed on file.
