Comments:
An unannounced renewal inspection was conducted today between 10:15 and 2:00 pm. There were 6 children (16 points) present initially with 1 caregiver who was the Substitute Provider. Twenty minutes later the Provider arrived. The children were observed playing on the playground, hand washing, during diaper change, eating lunches brought from home, watching a video and going down for nap. The entire physical plant, children's record, caregiver's records, household member's records, attendance logs, snack menu logs, evacuation logs, emergency plans, training logs and emergency supplies were inspected. There were no medications on site. Areas of non compliance were cited on the Violation Notice. Violations:
Standard #: 22VAC40-111-890
Description: Based on observation, the provider failed to ensure that when food is brought from home the food container shall be clearly labeled in a way that identifies the owner.

Evidence:
Six of six lunches did not have identification or dates.

Plan of Correction: We will remind parents to label and date children's lunches and if parents forget, we will label and date them.

Standard #: 22VAC40-111-100-A
Description: Based on a review of children's records, the provider failed to obtain documentation of a physical examination within 30 days after the first day of attendance.

Evidence:
Child # 3 does not have documentation of a physical within 30 days of attendance.

Plan of Correction: I will inform parent that child need to have a physical completed and I will place in child's file when I receive it from parent.

Standard #: 22VAC40-111-60-A
Description: Based on a review of children's records, the provider failed to ensure that children's records were up to date for each child enrolled.

Evidence:
1) Child #3 and #5's record's did not have documentation of names of persons other than the custodial parents who are authorized to pick up the child.

Plan of Correction: I will get the information from parents and place in children's files.

Standard #: 22VAC40-111-540-A
Description: Based on observation, the provider failed to ensure that feeding chairs will be used according to the manufacturer's instructions and when occupied by a child, a safety strap will be used and securely fastened.

Evidence:
During lunch, two children were sitting in chairs with feeding trays and safety straps; however, safety straps were not used to secure chidren.

Plan of Correction: We will remember to use safety straps.

Standard #: 22VAC40-111-690-D
Description: Based on observation, the provider failed to ensure that the diapering surface shall be separate from food preparation areas and not used for children's activities.

Evidence:
The diapering surface contained a tray with the children's lunch items.

Plan of Correction: The lunch items were immediately removed and the surface cleaned and sanitized.
