Technical Assistance:
The following was discussed with the provider:
- mulch under playground equipment
- injury records
- topical ointments

Comments:
An unannounced, mandated renewal inspection was conducted today. The inspector arrived at 1:00 PM and departed at 2:45 PM. Eleven children were in care with two caregivers upon arrival of the inspector. Transition and pick-up of half day children, circle time activities, outdoor play and nap time were observed. A sample size of nine household/caregiver records and six children's records were reviewed. Medication administration and injury records were reviewed.

An exit meeting was conducted with the provider prior to closure of the inspection.

Violations:
Standard #: 22VAC40-111-700-A
Description: Based on review of medication administration, the provider has not ensured that medication policies are followed.
Evidence:
1. There is emergency medication stored for possible administration to child 1. The child's record does not include written parental permission for the medication.
2. The provider verified that there was no written parental permission for the medication to be administered.

Plan of Correction: The provider responded with the following:
Written parental authorization will be obtained from the parent.
