Reviewed age appropriate eating practices and equipment needed.
A mandated renewal inspection was conducted today from 11:15am to 1:00pm. There were 6 children present (20 points) and 2 caregivers. Children were observed playing with manipulative toys,balls, stuffed animals, dancing to videos and singing songs, washing hands and preparing for lunch. Reviewed 2 children's files, 2 caregiver files, 4 household member files, attendance, menu, emergency evacuation log, and physical plant. Child#1 was missing 1 of 2 emergency contact's name, address and phone.
Plan of Correction: I will ask the parent for the information
Standard #: 22VAC40-111-180-A Description: Based on record review, provider did not obtain for each adult household member a current Report of Tuberculosis Screening form every two years from the date of the first screening
Household Member #1 did not have a current TB screening. Last documented one was 3/17/17.
