Discussion was held on the topics of diapering procedures, linens, fireplace inspection, feeding chairs and household members

Comments:
An unannounced renewal inspection was conducted today between the hours of approximately 9:40 am through 12:00pm with the provider on site. There were 11 children (28 points) directly supervised by 2 caregivers. A sample of 5 children's files and 3 staff members? files were reviewed. The children were observed during nap, reading, free toy play, outside play and lunch. Bathroom breaks and hand washing procedures were also observed. Interactions between the children and staff were positive. A walkthrough of the entire family day home was conducted. Cribs, the physical space, emergency supplies, evacuation drills, attendance records and the Emergency Preparedness Plan were reviewed. There were no medications today on site. Areas of non-compliance are identified in the violation notice. Violations:
Standard #: 22VAC40-111-180-A
Description: Based on record review, a subsequent tuberculosis (Tb) screening for a caregiver was not obtained every two years.

Evidence:
1) A current Tb screening was not documented for Staff #2.
2) The Tb screening documented for Staff #2 is dated 12/09/2016.

Plan of Correction: I will ask Staff #2 to get a new screening done.

Standard #: 22VAC40-111-240-A
Description: Based on observation, areas of the family day home were not maintained in safe and operable condition.
Evidence:
1) An outside lower area of the wall next to the screen door on the back porch showed signs of missing and rotten wood.
2) On the lattice fence, next to the garden area, protruding rusty nails were observed.
3) The landing area of a climbing toy leading to a slide was observed being wet posing a slip risk for children in care.

Plan of Correction: I will replace the rotten wood with a new piece of wood. Will replace the rusted nail with a new one.

Standard #: 22VAC40-111-280
Description: Based on observation, poisonous substances were not stored in an area inaccessible to children.
Evidence:
1) A bottle of a cleaning agent was stored in an unlocked cabinet under the sink in the bathroom used by children in care. Although the bottle was almost empty, there was still some liquid in it.
2) Four bottles of cleaning agents were stored on the floor of the laundry area. The laundry area is adjacent to the kitchen, and there is no door or any other
protective barriers that make the area inaccessible to children.

Plan of Correction: I will move the cleaning agents from the laundry room to a higher area that is inaccessible to children.
The bottle under the sink was removed and disposed of during inspection.

Standard #: 22VAC40-111-330-A
Description: Based on observation and interview, the family day home did not have a landline phone available.
Evidence:
1) A landline phone was not available during inspection.
2) Provider said they did not have a landline phone available.

Plan of Correction: I will buy a landline phone.

Standard #: 22VAC40-111-480-E
Description: Based on observation, equipment used by children was not free from hazards such as loose parts, sharp edges and rust.
Evidence:
1) Two climbing toy structures were observed with broken plastic that exposed sharp edges that pose a risk for the children in care: left side of a slide and outer covering of a tunnel.
2) The metal areas of two tricycles were covered in rust. One of the tricycles was missing the bell covering.

Plan of Correction: I will cover the sharp edges of the broken plastic with duct tape.
I will throw away the two tricycles.

Standard #: 22VAC40-111-500-C
Description: Based on observation, a climbing structure was not placed at least six feet from the perimeter of other play structures or obstacles.
1) A climbing structure (geo-dome climber) on the outdoor playground is placed at approximately 2 feet away, on its right side, from a wooden low wall.
2)The same structure is located at approximately 3 feet, on it?s left side, from another climbing toy structure.

Plan of Correction: I will move the equipment.

Standard #: 22VAC40-111-690-D
Description: Based on observation and staff interview, the diapering surface was not nonabsorbent and was not cleaned and sanitized after each use.
Evidence:
1) The diapering surface was observed with small rips which prevent it from being nonabsorbent.
2) Provider stated that she would clean the diapering surface with a Clorox wipe after each use. The provider also stated that she used the bleach solution at the end of the day, and not after every diaper change.
3) This process is not in compliance with the two-step cleaning and sanitizing procedure required.

Plan of Correction: I will buy a new diapering pad and replace it.
I will start using the correct procedure.
