Comments:
An unannounced monitoring inspection was conducted fro 9:52 am to 10:25 am. There were 9 children (25 points) in care of three caregivers. The children were observed playing with toys, getting bottle fed, coloring and doing a number learning activity. There were no medications on site. 5 children's, 1 caregiver and 1 household member record was reviewed. Areas of non compliance are listed in the violation notice. Standard #: 22VAC40-111-90-C
Description: Based on record review, the family day home did not obtain additional immunization records for children in care.
Evidence: Child # 1's first date of enrollment was April 2015. The date of the most recent immunization records obtained for Child # 1 were dated October 2015. Additional immunization records were not obtained for Child # 1 since that date.

Plan of Correction: I will get updated records.
