Standard #: 22VAC40-111-710 Description: Based on observation and record review, the family day home did not obtain written authorization for prescription medication, and the prescription medication was not maintained in it's original packaging.

2. The medication was not maintained in it's original labeled container dispensed by the pharmacy.
3. The provider did not obtain written authorization from the physician.
Plan of Correction: I will ask the parents to complete the form and bring in the original packaging for the medication.

1. Child # 4 had non prescription allergy medication on site.
2. The medication was not maintained in it's original packaging.

2. Child # 3's record did not have the authorization for emergency medical care completed by the parent.
