Comments:
A mandated Renewal inspection was conducted today, 10/10/2019 from 10:45 am to 12:30pm. There were 6 children present (14 points) with 1 caregiver. One child left during this inspection. Children were observed playing with cars and trucks, dancing, playing with manipulative toys, practicing their letters, and watching age appropriate tv programs. Reviewed 2 children's files, 1 caregiver file, 2 household member's files, attendance, menus, emergency evacuation log and supplies, crib compliance and physical plant. Violations:
Standard #: 22VAC40-111-920
Description: Based on observation, the menu of meals and snack provided by the family day home was not current for the week.
Evidence:
Provider stated the lunch today was fish stew, but the menu had Week 1 posted which was Chicken.

Plan of Correction: I will make sure it's current.

Standard #: 22VAC40-111-100-A
Description: Based on record review, provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.
Evidence:
Child#2 (Start date 11/1/17) did not have documentation of a physical.

Plan of Correction: I asked the parent this morning for it and they will bring it by next week.

Standard #: 22VAC40-111-40-A
Description: Based on observation, the provider did not ensure the total number of children receiving care at any one time did not exceed the maximum licensed capacity of the home.
Evidence:
There were 6 children present during this inspection, and the Provider stated none of them were residents of the home. The provider is licensed for 5 children.

Plan of Correction: This violation was corrected by a parent coming to the home during the inspection and picking up a child to take home.

Standard #: 22VAC40-111-240-A
Description: Based on observation, areas and furnishings of the family day home, inside and outside, are not binge maintained in a clean, safe, and operable condition
Evidence:
The rugs on the basement floor in areas used for child care all had obvious food, dirt and other debris on them. These types of materials were also observed under cribs, chairs, and couch.The

Plan of Correction: I can not clean because the baby will scream because she's scared of the vacuum. I will do it when she goes to bed.

Standard #: 22VAC40-111-480-E
Description: Based on observation, equipment and materials used by children shall be clean, nontoxic, and free from hazards.
Evidence:
The nap mats are ripped, torn and stuffing is observed coming out, which is a health hazard.

Plan of Correction: I will try to repair them before I buy new ones.
