Comments:
An unannounced monitoring inspection was conducted on October 10, 2019. The licensing inspector arrived at approximately 9:39 am and departed at 12:00 pm. At the time of the tour there were four children, ages eight months through four years old in care with the provider and two assistants totaling 12 points. The children were observed playing freely in the home, having music and movement and interacting with the caregivers. The sample size was six household member's records and five children's records. The family day home does not administer medication or offer transportation.

Violations:
Standard #: 22VAC40-111-780-B
Description: Based on record review, in one of five children's records reviewed, annually the provider did not review with the parent the emergency contact information required in 22 VAC 40-111-60 B 2 to ensure the information is correct and obtain the parent's signed acknowledgment of the review.

Evidence: The provider confirmed that the most recently signed review of emergency information for Child #4 was more than one year old and was dated 9/18/18.

Plan of Correction: I will have the parent correct it this afternoon at parent pick up.

Standard #: 22VAC40-111-120-B
Description: Based on record review, the provider did not ensure that each assistant's record contained all required information.

Evidence:

1. The provider confirmed that the record for Household Member/Assistant #3 did not contain documentation of the job title, date of employment, name, address and telephone number of a person to be notified in an emergency and documentation of orientation training.

2. The record for Household Member/ Assistant #2 did not include job title and the name, address and telephone number of a person to contact in the event of an emergency.

Plan of Correction: The records will be corrected today.

Standard #: 22VAC40-111-60-B
Description: Based on record review, the provider did not ensure that each child's record contained all required information.

Evidence: The provider confirmed that the following information had not been documented in each child's record:
Child #2-a complete work address for one parent
Child #4-one parent's home address
Child #5-first day of attendance and the policy number of the child's medical insurance

Plan of Correction: Provider will contact the parents and have them to update the records.

Standard #: 22VAC40-111-320-A
Description: Based on observation and inspection of the family day home, the provider did not ensure that small electrical appliances such as, but not limited to, curling irons, toasters, blenders, can openers, and irons shall be unplugged unless being used by the caregiver or with children under close supervision.

Evidence: The provider confirmed that there were small appliances plugged into outlets in the kitchen that were not currently being used by the provider.

Plan of Correction: The provider unplugged the appliances during the inspection and put outlet covers in the outlets.

Standard #: 22VAC40-111-320-B
Description: Based on observation and inspection of the family day home, the provider did not ensure that child-resistant protective covers shall be installed on all unused electrical outlets and surge protectors when children under five years of age are present.

Evidence: The provider confirmed that there were three uncovered electrical outlets in the kitchen. All of the children in care were under five years of age.

Plan of Correction: All outlets were covered.
