Comments:
An unannounced monitoring inspection was conducted today between 9:00 am and 12:00 pm. There were 5 children (12 points) with 2 caregivers. The physical plant, attendance logs, evacuation logs, emergency plans, cribs, bottles, children's records,caregiver's record, household member's records and emergency supplies were inspected. There were no medications on site. The children were observed playing with puzzles, looking at books and having morning snack. Areas of non compliance were cited in the Violation Notice. Violations:
Standard #: 22VAC40-111-750-A
Description: Based on observation and review of children's records, the requirements for the use of topical skin products were not met.

Evidence:
The provider had one diaper cream that was not labeled and there was not a written parent authorization for the diaper cream.

Plan of Correction: I will remind parents to label the diaper creams and I will have the parent fill out an authorization form.

Standard #: 22VAC40-111-890
Description: Based on observation, the provider failed to ensure that when food is brought from home, the containers shall be clearly labeled.

Evidence:
There was a lunch box and 3 containers of children's food brought from home, but were not labeled.

Plan of Correction: I will remind parents to label all food brought from home, and if they forget I will label it.

Standard #: 22VAC40-111-960-E
Description: Based on observation, the provider failed to ensure that bottles are labeled with the child's full name and the date.

Evidence:
There were three bottles in the refrigerator that were not labeled.

Plan of Correction: I will remind parents to label bottles with the child's full name and the date.

Standard #: 22VAC40-111-60-A
Description: Based on a review of records, the provider did not maintain up to date records for each child enrolled.

Evidence:
1) Child #3's record did not have documentation of an address for the first emergency contact.
2) Child #5's record did not have documentation of an address for the second emergency contact.
3) Child #5's record did not have the Information for Parent Form signed by a parent, a Policy for Medication Form signed by the parent, an Emergency Medical Authorization signed by the parent or an Emergency Preparedness and Response plan signed by the parent.
4) Child #5's record was missing the names of person other than the custodial parents who are allowed to pick up the child.
5) Child #1's record did not have documentation of an insurance policy number.

Plan of Correction: I will get information from parents and place in files.

Standard #: 22VAC40-111-280
Description: Based on observation, the center failed to ensure that potentially poisonous substances shall be stored in areas not accessible to children.

Evidence:
There was diaper cream stored under the changing table and accessible to children.

Plan of Correction: The diaper cream was removed from basket under the changing table during the inspection. We will store diaper cream away from .children

Standard #: 22VAC40-111-680-B
Description: Based on observation, the provided failed to ensure that children's hands are washing with liquid soap and running water before eating.

Evidence:
Caregiver #1 placed two children in highchair for morning snack, the assist wiped the children's hand with a paper towel.

Plan of Correction: We will wash the children's hands before eating.

Standard #: 22VAC40-665-240-C-2
Description: Based on observation, the provider failed to ensure that electrical outlets will have protective covers that are of a size that cannot be swallowed by children.

Evidence:
In the napping room there were three electrical outlets without protective covers.

Plan of Correction: I will replace outlet covers.
