Comments:

An unannounced monitoring inspection was conducted today from 10:00am-1:00pm. There were 2 children in care and one resident child (7 points) supervised by 1 direct care staff. The home, 1 staff record, 2 children?s records, 4 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed engaging freely, playing outside, and napping. There was a sufficient amount of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. Description:	Based on observation, the provider did not ensure that the posted menu was for the current one-week period. Evidence: The posted menu was dated February 4-9.

Description:	Based on record review, 1 of 2 children's records did not contain documentation of all required information. Evidence: Child #1's record did not contain documentation of results of the health examination for the child as required by 22 VAC 40-111-100, and documentation of the review of the child's emergency contact information as required by 22 VAC 40-111-780 B. The most recent documented review of the child's emergency contact information was dated 3/16/2018.
Plan of Correction:	Parent has been contacted and will bring a copy of the child's physical today. I will have the parent sign the emergency contact information today.

Description:	Based on record review, 1 of 4 household member records did not contain documentation of a current report of TB screening form prior to coming into contact with children. Evidence: Household member D's record did not contain documentation of a current report of TB screening form.
