Comments:
An unannounced renewal inspection was conducted today from 1:20pm-3:15pm. There were 10 children (32 points) supervised by 3 direct care staff. The home, 3 staff records, 5 children?s records, 3 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed napping. Bottle-feeding procedures were also observed. There was a sufficient amount of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. Areas of non-compliance are identified in the Violation Notice.

Standard #: 22VAC40-111-60-B
Description: Based on record review, 4 of 5 children's records did not contain documentation of all required information.

Evidence:
1. Child #4's record did not contain documentation of either parent's work telephone number, full addresses for either of the two required emergency contact persons; and parent's signed acknowledgement of the receipt of the information regarding policies for the administration of medications, and provisions of the family day home's emergency preparedness and response plan.
2. Child #5's record did not include documentation of a complete address for the second emergency contact person, the medical insurance policy number, and parent's signed acknowledgement of the receipt of the information required by 22 VAC 40-111-70.
3. Child #8's record did not include documentation of a complete address for the first emergency contact person.
4. Child #10's record did not include documentation of parent's signed acknowledgement of the receipt of the information required by 22 VAC 40-111-70 and provisions of the family day home's emergency preparedness and response plan.

Plan of Correction: I will get all missing information from parents.
