Comments:
An unannounced monitoring inspection was conducted today to modify the license to reflect the new address. There are 2 children in care (4 points) with one caregiver.
Two children's records were reviewed. See violation notice for missing information.
The children were sleeping during today's inspection. Emergency supplies, emergency evacuation drills, refrigerator thermometer, phone landline, and landline telephone are all in compliance. Standard #: 22VAC40-111-60-B
Description: Based on review of records, one child's record is incomplete.

Evidence:

1 of 2 records reviewed was missing documentation of a complete address for a second emergency contact. (Child 1)

Plan of Correction: I will ask the parent to provide the address.

Standard #: 22VAC40-111-70-A
Description: Based on a review of records, parents have not been provided in writing, the Provisions of the Emergency Preparedness Plan.

Evidence:
The provisions of the emergency preparedness plan do not reflect the new emergency relocation destination.

Plan of Correction: I have already given the form to the parent to sign, they have not returned it to me yet.
I will ask them for it again.
