Technical Assistance:
Discussion was held on the topics of current enrollment, current staffing, and a possible relocation of the child care sometime next year. Forms were emailed earlier and a discussion was held on ensuring that the child care continues to operate within the residence of the Provider both before and after any future move. Additional discussion was on employment. While I can confirm the current status of license and the operating hours as indicated online by what was completed within the license application, the department cannot issue a statement based on the Provider's full-time or part-time employment status.

Comments:
The following inspection took place between the hours of 9:30 am through 12 pm (noon). Today, I met with the Provider on site at the Family Day Home (FDH) located in Sterling Park, Virginia. At the start of this unannounced monitoring inspection there were a total of 4 children observed (8 points) with 1 caregiver. A total of 4 children?s files, 1 staff file, and 2 household member?s files were reviewed. The children were engaged in the following: morning snack, toy play, song, music, dance, learning activities, large motor skills with a hula hoop, bathroom breaks, hand washing, lunch, and other organized activities. Violations:
Standard #: 22VAC40-111-140-B-3
Description: Based on review, 1 of 1 staff file indicated that the certification in cardiopulmonary resuscitation (CPR), as appropriate to the age of the children in care had expired.

Evidence -
1. On the date of inspection, the most recent certification in CPR for Staff #1 had the expiration date of 08/31/2019.
2. No other adult person on site was certified in CPR.

Plan of Correction: Staff #1 shall obtain a new certification in CPR.

Standard #: 22VAC40-111-140-B-4
Description: Based on review, 1 of 1 staff file indicated that the certification in first aid had expired.

Evidence -
1. On the date of inspection, the most recent certification in first aid for Staff #1 had the expiration date of 08/31/2019.
2. No other adult person on site was certified in first aid.

Plan of Correction: Staff #1 shall obtain a new certification in first aid.

Standard #: 22VAC40-111-180-A
Description: Based on review, 1 of 1 staff file and 2 of 2 household member files, indicated that the Provider did not obtain for each caregiver and adult household member a current Report of Tuberculosis (TB) Screening form (in accordance with the requirements in 22 VAC 40-111-170), every two years.

Evidence -
1. On the date of inspection, the most recent TB documentation on file for Staff #1 was dated - 08/24/2017.
2. The most recent TB documentation on file for Household Member #1 was dated - 09/12/2017.
3. The most recent TB documentation on file for Household Member #2 was dated - 08/29/2017.

Plan of Correction: Updated TB documentation shall be obtained for Staff #1, Household Member #1, and Household Member #2.
