Technical Assistance:
Discussed the following issues with the Provider:
- custodial issues and the rights of the parents
- changes in health insurance for children in care

Comments:
An unannounced monitoring inspection was conducted today. The physical plant, 5 caregiver/household member records, 5 children?s records, evacuation drills, emergency supplies, and policies were inspected between 12:15-1:30pm. There were 7 children, totaling 19 points, directly supervised by 2 staff. The Provider is not currently certified in MAT, and there are no children with any medications needs at this time. Today, the children listened to music, played outside, ate lunch and rested during naptime. There was an abundant supply of books, toys, and materials for the children. The home was clean and organized. For lunch, the children were served ground beef, brown rice, peas and carrots, oranges, whole wheat bread and milk. Standard #: 22VAC40-111-90-C
Description: Based on file review, 1 out of 5 children's records does not have additional immunizations every six months up to the age of 2 years.
Evidence
1. Child #5 is 15 months old. The most recent immunization record was completed when the child was 8 months old.

Plan of Correction: The parent told the Provider that the child was a dr appt in the next week. This will be brought to the FDH.

Standard #: 22VAC40-111-170-A
Description: Based on file review, 2 adult household members do not have documentation of a TB test/screening.
Evidence
1. Caregiver/household members #4 and #5 do not have a completed TB test/screening.

Plan of Correction: They will complete this next Tuesday.

Standard #: 22VAC40-191-40-D-4-a
Description: Based on file review, 2 adult household members do not have a completed sworn disclosure statement (SDS).
Evidence
1. Caregiver/household members #4 and #5 do not have a completed SDS on file.
