Comments:
An unannounced monitoring inspection was completed today. The physical plant, 3 caregiver/household member files, 2 children's files, caregiver training, child nutrition, children's activities, emergency procedures, drills and supplies were inspected between 11:50- 1:15pm. There were 2 children, totaling 4 points, under the supervision of the Provider. During the inspection, children were observed enjoying free play, reading book and interacting with the Provider. Earlier today, the children had a visit from Fairfax County Early Literacy Outreach Program. Through this program, the children listen to a story and then are given a book to keep. Today, the children were served beans with rice, corn, strawberries and milk. The Provider is not currently certified in MAT (medication administration training) and there are no children with any medication needs at this time. Plan of Correction: The Provider will ask the parent to update and sign the file.

Standard #: 22VAC40-111-60-B
Description: Based on file review, 1 out of 2 children's records does not have all required information.
Evidence
1. Child #1's record is missing the following information:
- Name, home address, and telephone number of each parent who has custody.
- Name, address and telephone number of each custodial parent's place of employment.
- Name, office address and telephone number of the child's physician.
- Name, address and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reached.
- Information on allergies and intolerance to food, medication, or any other substances, and actions to take in an emergency situation.
- Name and policy number of the child's medical insurance, if applicable.
- Written authorization for emergency medical care should an emergency occur and the parent cannot be located immediately.

Plan of Correction: The Provider will ask the parent to complete this when she comes to pick up her son today.

Standard #: 22VAC40-111-70-A
Description: Based on file review, 1 out of 2 children's records does not have all required information signed by the parent before the child's first day of attendance.
Evidence
1. Child #1 does not have a signed "Information for Parents", Medication Policy, Liability Insurance Statement, or "Provisions to the Emrgency Preparedness Plan" on file.

Plan of Correction: The Provider has this paperwork and will have the parent review and sign it today.

Standard #: 22VAC40-111-90-A
Description: Based on file review, 1 out of 2 children's records does not have documentation that the child has been adequately immunized before the child attends the FDH.
Evidence
1. Child #1 began receiving care yesterday (Oct 9th) and does not have documentation of an immunization record.

Plan of Correction: The Provider will ask the parent to bring this to the FDH.
