Comments:
An unannounced monitoring inspection was conducted on 10/04/2019 from 2:15 pm - 3:05 pm. At the time of entrance there were six children in care with two caregivers. The ages of the children counted for a total of 14 points. Children were observed during nap. Records were reviewed for six children and two caregivers.
The violation is listed on the violation notice and was reviewed with the provider at the conclusion of the inspection.

Violations:
Standard #: 22VAC40-111-240-A
Description: Based on observation, the provider failed to ensure that outside areas of the family day home shall be maintained in a safe condition.

Evidence:
1. The plastic slide was not attached to the climber. When the inspector checked the slide it easily lifted up from the top which exposed the sharp point of two screws.
a. Injury could occur if the slide moved or detached while in use by a child.

Plan of Correction: The provider stated that the climber would be removed from the outdoor play area.
