54.1-3408 Provider must be MAT certified to administer prescription medication.
An unannounced renewal inspection was conducted today between the hours of 9:05am through 10:30am with the provider on site. There were 4 children (14 points) with the provider and assistant. A sample of 3 children's files and 4 staff/household member's files were reviewed. The children were engaged in the following: toy play, hand washing and eating snack. Interactions between the children and caregivers were positive. The physical space, emergency supplies, evacuation drills, attendance records and the Emergency Preparedness Plan were reviewed. Standard #: 22VAC40-111-940-B Description: Based on observation, the provider did not have an operable thermometer available to monitor refrigerator and freezer compartments temperatures.
1. The provider had a thermometer, but it was not operable.
Plan of Correction: I will get new batteries for the thermometer.
Standard #: 22VAC40-111-100-A Description: Based on review of 3 children's records, the provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.
1. Child #3 began with the provider on 8/27/2019 and did not have record of a physical examination within 30 days after attendance.
Plan of Correction: The provider requested a copy of the physical during the inspection.
