An unannounced monitoring inspection was conducted between the hours of 10:00 am and 1:00 pm. When the inspector arrived the licensee was on the way back from a field trip with the children in care. When the provider arrived, there were 3 children, totaling 11 points with 1 caregiver. All 3 children's records were reviewed in addition to 2 caregiver records and 2 pet records.

Standard #: 22VAC40-111-320-A Description: Based on observation the licensee did not ensure small electrical appliances were unplugged.
Evidence:
A toaster was plugged into an electrical outlet in the kitchen.Plan of Correction: The following corrective action was provided by the licensee during the exit interview:
The licensee unplugged the toaster during the inspection.
Standard #: 22VAC40-191-60-B Description: Based on a record review, the center did not ensure all caregivers and household members had the required background checks.
Evidence:
Caregiver #1 and #2 did not have the most current sworn statement completed.Plan of Correction: The following corrective action was provided by the licensee during the exit interview:
The licensee will print the most recent sworn statement and both caregivers will complete the sworn statements.
