Comments:
An unannounced monitoring inspection was conducted today from approximately 11:25am to 2:45pm. 12 children (34pts) were present with 3 caregivers. The physical space, 4 children's records, Provider/assistant/household member records, evacuation drills, the menu and required posted documents were reviewed. No medications were on site. Children were observed finishing bottles, playing in small groups with puzzles and blocks as well as interacting with caregivers. The space contained age appropriate materials and activities. Violations:
Standard #: 22VAC40-111-960-B
Description: Based on observation, an infant who could not hold their own bottle, was not picked up and held for bottle feeding.

Evidence: Upon arrival of the Licensing Inspector, a bottle was propped, with a blanket, for Child #2 while the child sat in a bouncy seat.

Plan of Correction: We will only hold infants when feeding them.

Standard #: 22VAC40-111-100-A
Description: Based on record review, 1 out of 4 childrens' records did not contain documentation of a physical examination within 30 days after the first day of attendance.

Evidence: Child #1 started on 8/19/2019. No physical examination documents were available for review.

Plan of Correction: I will get the information from the parents right away.

Standard #: 22VAC40-111-50-B
Description: Based on record review and interview with the Provider, the Provider did not maintain a record for one out of 2 assistants present.
Evidence: The Provider stated Assistant #1 came to help because the regular assistant was out sick today. The records for Assistant #1 were in progress and not available for review.

Plan of Correction: I will complete the file for the assistant and send the information to the licensing inspector.

Standard #: 22VAC40-111-60-B
Description: Based on record review, 1 out of 4 childrens' records were incomplete.

Evidence: Child #1 only had emergency information for one contact. Two designated persons to contact are required in case of an emergency, if the parent cannot be reached. Child #1 was missing the policy insurance number. The name of the medical insurance company was noted, but the policy number was missing.

Plan of Correction: I'll get the information right away.

Standard #: 22VAC40-111-520-C
Description: Based on observation and inspection of cribs, 3 out of 7 were not labeled with the date they were manufactured and no documentation was available for review.

Evidence: Three cribs did not have a board base with a date manufactured. The three cribs had a wire base and no printing anywhere else on the crib. The Provider stated she had documentation from when the cribs were purchased, with their manufactured dates, and had shown it to a previous inspector. However, the documentation was unavailable for review today.

Plan of Correction: I will look for the information and if I cannot find it, I will buy new cribs.
