Comments:
An unannounced monitoring inspection was conducted from 12:00pm-1:35pm. There were 9 children (23 points) in the direct care and supervision of the Provider and two Assistant Providers. A sample size of 4 children's records, 3 caregiver records, 1 household member record, the first aid kit, emergency materials, evacuation drills, the daycare space, and the playground were reviewed. The children were observed dancing, helping carve pumpkins, playing with toys, playing outside on the playground, eating lunch, washing hands, having their diapers changed, toileting, and preparing to lay down for their nap time. Standard #: 22VAC40-111-180-A
Description: Based on record review, the Provider did not obtain an updated tuberculosis screening for 2 caregivers and one household member.
Evidence:
1. Staff #1's last tuberculosis screening was dated 9/13/2017.
2. Staff #3's last tuberculosis screening was dated 8/31/2017.
3. Household Member #1's last tuberculosis screening was 7/24/2017.

Plan of Correction: We will get new TB screenings.

Standard #: 22VAC40-111-200-C
Description: Based on a review of Assistant caregiver records, 1 out of 2 records did not contain documentation of the orientation.
Evidence:
1. Staff #3 (DOH: 8/7/2017) did not have documentation of the orientation on file.

Plan of Correction: I will look for it or give a new one.

Standard #: 22VAC40-111-320-B
Description: Based on observation, not all unused electrical outlets had protective covers.
Evidence:
1. Two electrical outlets located on the wall where the cots are located did not have protective covers.

Plan of Correction: Corrected during inspection.

Standard #: 22VAC40-111-690-D
Description: Based on observation, the diaper changing pad is non-absorbent.
Evidence:
1. The diaper changing pad had two tears in it making it a nonabsorbent surface.

Plan of Correction: I will buy a new one.
