Reviewed requirements for shelter in place emergency drills each year.
- Comments:

An unannounced monitoring inspection was conducted today. The provider and two assistant caregivers were caring for 5 children, totaling 10 points. Children's records, caregiver:child interactions, the indoor/outdoor physical plant, emergency preparedness items, health records, caregiver training, and children's nutrition were inspected today. The provider reported that no children's medications were on site today.
The children were observed playing with toys, interacting with each caregiver, and eating lunch. The children were served a hot lunch of chicken nuggets, rice, mixed vegetables, and milk.

Standard #: 22VAC40-111-100-A Description: Based on a review of children's health records, it was determined that the provider did not obtain documentation of a physical exam for one child within 30 days of the child's first day of attendance.
Evidence:
Child A entered care on 4/19/19. As of the date of this inspection, the provider has not obtained documentation of a completed physical exam for the child.Plan of Correction: I will remind the parent to bring the completed physical form completed and signed by the doctor.
