Comments:
An unannounced monitoring inspection was conducted today between the hours of 1:30pm through 3:10pm with the provider on site. There were 10 children (25 points) with two caregivers present. A sample of 5 children's files and 3 caregiver/household member's files were reviewed. The physical space, emergency supplies, evacuation drills, attendance records, and the Emergency Preparedness Plan were also reviewed. The children were engaged in the following: naptime, toy play, and coloring/drawing. Areas of non-compliance are identified in the violation notice. Standard #: 22VAC40-111-830-A
Description: Based on record review, emergency evacuation procedures were not practiced monthly.
Evidence:
A fire drill was not documented during the months of December 2018 and February 2019.

Plan of Correction: I will make sure to write down when fire drills are completed. We do fire drills when we go outside.

Standard #: 22VAC40-111-60-B
Description: Based on record review, 1 of 5 children's records did not contain the required information.
Evidence:
The record for Child #1 did not contain the name and policy number of the child's medical insurance.

Plan of Correction: I will text the parents and ask for the information today.
