Comments:
An unannounced monitoring inspection was conducted on September 16, 2019 from approximately 9:15 am to 11:40 am. Another caregiver was present with four children at the onset of inspection for a total of twelve points. Provider arrived back to the home at approximately 9:45 am. Two caregiver and three children's records were reviewed.

Please complete the columns for ?description of action to be taken? and ?date to be corrected? for each violation cited on the violation notice, and then return a signed copy to the licensing office within 5 calendar days of receipt. Violations:
Standard #: 22VAC40-111-920
Description: Based on review, provider did not ensure the current one-week menu is posted or given to parents.

Evidence: The posted menu was dated "Sept 9 - 13" which is not the current week.

Plan of Correction: Menu was posted same day.

Standard #: 22VAC40-111-120-B
Description: Based on review, provider did not ensure substitute provider record has required documentation.

Evidence: The record for caregiver #2 did not have verification of age, job title, start date of employment or volunteering, emergency contact information, two reference checks and education/experience required for the position.

Standard #: 22VAC40-111-50-A
Description: Based on review and observation, provider did not keep a written record of children in attendance each day.

Evidence: Seven children were signed in as present on September 16th however four children were observed in the home. Provider stated that children had left for school.

Plan of Correction: The two children was sign out.

Standard #: 22VAC40-111-60-B
Description: Based on review, provider did not ensure 1 of 3 children's records has documentation of required information.

Evidence: The record for child #1 (start date 8-31-2019) did not have father's work address, father's work phone number, any chronic physical problems and any previous programs attended.

Plan of Correction: The information for father was done.

Standard #: 22VAC40-111-140-B-3
Description: Based on interview, provider did not ensure when a caregiver is left alone with children that the individual is CPR certified.

Evidence: Caregiver #2 was observed alone in the home with three enrolled children from approximately 9:20 am to 9:46 am. There is not documentation of caregiver #2 being certified in CPR. Provider stated she did not know if the individual is currently certified. Provider confirmed she had left the home to go a nearby store.

Standard #: 22VAC40-111-140-B-4
Description: Based on interview, provider did not ensure when a caregiver is left alone with children that the individual is first aid certified.

Evidence: Caregiver #2 was observed alone in the home with three enrolled children from approximately 9:20 am to 9:46 am. There is not documentation of caregiver #2 being certified in first aid. Provider stated she did not know if the individual is currently certified. Provider confirmed she had left the home to go a nearby store.

Plan of Correction: First aid was taken on 9/26/2019.

Standard #: 22VAC40-111-200-A
Description: Based on review, provider did not ensure the substitute received orientation training by the end of her first week of assuming job responsibilities.

Evidence: The record for caregiver #2 does not have documentation of orientation training. Provider stated caregiver #2 served as substitute on 5-2-2019.

Standard #: 22VAC40-111-240-A
Description: Based on observation, provider did not ensure areas and furnishings of the home are maintained in a clean, safe and operable condition. Unsafe conditions shall include, but not be limited to, the presence of poisonous plants; tripping hazards; unstable heavy equipment, furniture, or other items that a child could pull down on himself; splintered, cracked, or otherwise deteriorating wood; chipped or peeling paint; visible cracks, bending or warping, rusting, or breakage of any equipment; head entrapment hazards; and protruding nails, bolts, or other components that could entangle or could snag skin.

Evidence: A pack of cigarettes were observed sitting on the dining room table in the area occupied by and within reach of children.

Plan of Correction: The cigarettes were removed to another place

Standard #: 22VAC40-111-690-E
Description: Based on observation, provider did not ensure soiled disposable diapers and wipes are disposed of in a leak-proof or plastic-lined storage system that is either foot operated or used in such a way that neither the caregiver's hand nor the soiled diaper or wipe touches the exterior surface of the storage system during disposal.

Evidence: After changing a child's diaper, provider was observing placing the soiled diaper in a plastic bag and then throwing the diaper/bag in the front yard.

Plan of Correction: A proper disposal was placed in area.

Standard #: 22VAC40-191-60-C-2
Description: Based on review and interview, provider did not ensure an individual is denied continued employment if there is not a central registry check finding within 30 days of beginning employment.

Evidence: The record for caregiver #2 did not have a completed central registry check and the individual was present with children during inspection while provider left the home. Provider stated caregiver #2 served as substitute on 5-2-2019.

Plan of Correction: The paper work was sent off.
