Comments:
A Subsidy Health and Safety inspection was conducted on September 16, 2019 to review supplemental health and safety requirements. Another caregiver was present with four children at the onset of inspection for a total of twelve points. Provider arrived back to the home at approximately 9:45 am. Two caregiver and three children records were reviewed. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Description: Based on observation and interview, provider did not ensure all caregivers complete Virginia Preservice Training for Child Care Staff sponsored by the Department of Social Services.

Evidence: The record for caregiver #2 did not have documentation of Virginia Preservice Training. Provider stated caregiver #2 has served as a substitute on May 2, 2019 and September 16, 2019.

Description: Based on interview, provider did not ensure the written emergency preparedness plan has documentation of all required components.

Evidence: Provider stated she did not have written procedures to address continuity of operations to ensure that essential functions are maintained during an emergency.
