Comments:
An unannounced non-mandated monitoring inspection was completed as a part of the requirement during the conditional license period. The inspector arrived at the provider's home at 9:15 AM and departed at 11:45 AM. There were 4 children present with one provider. The point total was 15. The provider's training record, pet vaccination records, and all new children's records were reviewed. There were no medications present and the provider does not transport children at this time. The inspector observed nap time, the first aid kit, emergency drill documentation and supplies, attendance documentation, and free play. There was discussion with the provider about the following: subsidy requirements, the licensing inspection process, and feeding.

Description: Based on observation and discussion with the provider, the provider failed to ensure that when meals or snacks are provided by the family day home, the menu for the current one-week period shall be dated, be given to parents or posted or placed in an area accessible to parents, list any substituted food, and be kept on file one week at the family day home.

Evidence:

1. The LI did not see a posted menu and inquired about the menu. The provider stated that she did not have a menu printed out for parents nor posted.

Plan of Correction: The provider will make sure that the menu is printed out.
