Comments:
An unannounced focus inspection was completed today between 11:00 am and 12:30 pm. The inspector picked up verification that letters were sent to parents regarding Sanction; letters included signatures of parents for children enrolled at the time of the incident. There were 9 children (19 points) present with 2 caregivers

Violations:
Standard #: 22VAC40-111-900-B
Description: Based on observation, the provider allowed children to eat or drink while walking, running, playing, lying down or riding vehicles.

Evidence:
Child #2 was lying down and drinking from bottle.

Plan of Correction: I will remind assistants that children can drink from a bottle while lying down.

Standard #: 22VAC40-111-90-A
Description: Based on a review of children's records, the provider failed to ensure that all children enrolled have documentation of immunization on the first day of attendance.

Evidence:
Child #9 does not have documentation of immunizations on the first day of attendance.

Plan of Correction: I will ask parents for shot record today.
