Technical Assistance:
An exit interview was conducted with the provider to discuss the results of today's inspection. Consultation was given regarding accessing the department's website for updated information and additional forms.

Comments:
An unannounced renewal inspection was conducted today, October 18, 2019 to determine the provider's compliance with licensing standards. The inspection was initiated at 1:45pm and concluded approximately 3:30pm. In attendance were 4 children totaling 11 points being supervised by the provider. Children were observed playing with toys, using the restroom, watching an educational program, and interacting with provider. Children's books, toys, and equipment were inspected and found to be age appropriate and in operable condition. All rooms of the home and outdoor areas were inspected with no hazards found.

The family day home had the required postings displayed on the wall upon entering the children's room. The first aid kit, fire extinguisher, and emergency supplies were inspected. Four children's records and provider records were inspected with the majority of the required information found. The following was also inspected: fire drill log, shelter-in-place log, parent agreements, emergency preparedness plan, daily attendance log, and other pertinent information.

Note: The provider has moved to a new location in which the entire home was inspected including outdoor areas to modify the license.

Violations:
Standard #: 22VAC40-111-100-A
Description: Based on the inspection of administrative records, the required documentation was not obtained.

Evidence:
During the inspection of four children's records it was discovered child #4 did not have documentation of a physical examination on record. Licensing standards require the provider to obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.

Plan of Correction: Per provider: I will have the parent to get copies and place them in the child's record.

Standard #: 22VAC40-111-30-A
Description: Based on the inspection of administrative records, the family day home did not follow the terms of the current license.

Evidence:
During the inspection of administrative records, it was determined the family day home did not follow the correct procedures in notifying the Department prior to moving the family day home to a new location.

Plan of Correction: Per provider: If I move again, I'll be sure to notify the agency as soon as possible.

Standard #: 22VAC40-111-90-A
Description: Based on the inspection of administrative records, the required documentation was not obtained.

Evidence:
During the inspection of four children's records it was discovered child #4 did not have documentation of the child's immunizations on record. Licensing standards require the provider to obtain documentation that the child has been adequately immunized before the child may attend the family day home.

Plan of Correction: Per provider: I will get a copy of the immunizations from the parent and place them in the child's record.

Standard #: 22VAC40-111-170-A
Description: Based on the inspection of administrative records, the required TB (tuberculous) test or screening documentation was not available to review.

Evidence:
During the inspection of administrative records, it was discovered household member #2 did not have documentation of a TB test or screening in the record.

Plan of Correction: Per provider: I will have a TB test completed with the results placed in the record.
