Comments:
An unannounced Subsidy Health & Safety inspection was conducted today, October 18, 2019 to determine the family day home's compliance with subsidy requirements. The inspection was initiated at 1:45pm and concluded at approximately 3:30pm. All subsidy requirements were reviewed and overall compliance determined. Administrative records, all areas in the home used by children, and outdoor play areas were inspected.

A licensing renewal inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. See attached notice for violations found during this inspection.

Standard #: 22VAC40-665-400-B-7
Description: Based on the inspection of administrative records, the family day home did not have the required Continuity of Operations procedures available to review.

Evidence:
During the inspection of administrative records, it was discovered the family day home did not create the required Continuity of Operations procedures to ensure essential functions are maintained during an emergency.

Plan of Correction: Corrected on site.

Standard #: 22VAC40-665-410-A-3
Description: Based on the inspection of administrative records, the family day home did not have documentation that the required lockdown drill was practiced.

Evidence:
During the inspection of administrative records, it was discovered the family day home did not have documentation that the lockdown drill for the year 2018 was conducted. Subsidy health and safety requirements stipulate for the family day home to practice the lockdown drill annually with documentation maintained to review.

Plan of Correction: Per provider: I will make sure the lockdown drill is practiced and recorded before the end of this year.
