Comments:

An unannounced renewal inspection was completed on 10/18/2019. There were 10 children present from two months through three years old with the licensee and an assistants supervising. The point total was 25. Five enrolled children's records were reviewed, the provider's record was reviewed, and the assistant's records were reviewed. The inspector arrived at the provider's home at 7:20 am and departed at 11 am. The children were observed during: arrival, nap time, breakfast, free play time, and circle time.

Standard #:	22VAC40-111-60-A
Description:	Based on review of five children's records, the licensee failed to ensure that all required information was obtained. Evidence: 1. Child # 1 did not have the health insurance policy number documented.
Plan of Correction:	The licensee will have the parent to obtain the policy number and will monitor records.
Standard #:	22VAC40-111-70-A
Description:	Based on review of five children's records, the licensee failed to ensure that before all child's first day of attendance that parent shall be provided in writing child's medical authorization form, information for parents, liability insurance, and provision for emergency. Evidence 1. Child # 5 did not have documentation that the parent received in writing the parent information.
Plan of Correction:	The licensee will have the parent to sign a copy and a copy will be given to the parent to keep.
Standard #:	22VAC40-111-90-C
Description:	Based on review of five children's records, the licensee failed to ensure that all immunizations were updated as required by the standard. Evidence: 1. Child # 2 was enrolled on 5/28/2019 and did not have immunization updated every 6 months until the child was 2 years old. The most recent immunization record on file was from 5/7/2018. 2. Child # 1 was enrolled on 9/17/2018 and did not have immunization updated every 6 months until the child was 2 years old. The most recent immunization record on file was from 6/28/2018.
Plan of Correction:	The licensee will have the parents to obtain a updated copy and will monitor records.
Standard #:	22VAC40-111-570-A
Description:	Based on observation, the licensee failed to ensure that the caregiver does not exceed 16 points by using the point system. Evidence: 1. Observed at 8:11 A.M. the licensee was at 17 points and at 8:28:A.M. the licensee was at 19 points with no additional caregiver present. The assistant arrived at 8:50 A.M.
Plan of Correction:	The licensee stated that the assistant was running late and is scheduled for 8:30 A.M. The licensee will look at the schedule to ensure that she is within points for all caregivers in the future.
