Technical Assistance:
Discussed that only emergency contact information is required to be reviewed with the parents annually unless policies and procedures change, then disclosure needs to be acknowledged.

Comments:
An unannounced Monitoring inspection was conducted this date. A total of 6 children (15 pts) with Provider and 1 Assistant present today. When the Inspector arrived the children had completed a seasonal art project out of construction paper and wooden sticks that made a Jack O'Lantern to hang. The home was decorated with large letters of the alphabet, numbers, animals and pictures of the children. The children were observed playing with musical instruments and singing and dancing with each other. Hand washing and diapering procedures were observed as the children transitioned to lunch. Lunch today was baked chicken, rice and lentils with grapes. Staff were observed sitting with the children while they ate and encouraged independence with using utensils. New equipment on site was observed which included a large interactive school bus that allowed for tactile stimulation and exploration. Positive interaction was observed between staff and children. Observation of the crib room and other small programming room was made and the outdoor playground area. A total of 6 children's records were reviewed today, both staff and one household member. No medications on site and no identified food allergies. This inspection began at approximately 10:30am and ended at 12:40pm. Thank you for your cooperation today. Description: Based on child record review, documentation was not maintained in one child record.
Evidence: Child # 4's record did not contain the original physical examination and immunizations obtained at the time of enrollment.

Plan of Correction: This documentation was obtained but files were thinned and I did not know I needed to maintain this. Ongioing, I will maintain all information for two years past the child's last day.
