Discussion was held on the topics of current enrollment, and upcoming timeline for a household member and the search of the central registry.
The following inspection took place between the hours of 8:30 am through 10:30 am. Today, I met with the Provider on site at the Family Day Home (FDH) located in Fairfax, Virginia. At the start of this unannounced monitoring inspection there were a total of 2 children observed (6 points) with 1 caregiver. A total of 3 children?s files, 1 staff file, and 3 household member?s files were reviewed. The children were engaged in the following: naming - shapes, objects, and colors; bottle feeding, snack time, toy play, video learning, story time, diapering, and other organized activities. Standard #: 22VAC40-111-800-B Description: Based on review and interview, the Provider did not review the Emergency Preparedness and Response Plan (EP&RP) at least annually and document in writing each review and update (if applicable) to this plan.
1. On the date of inspection (10/21/2019), the most current documented review of the EP&RP was dated - 09/01/2018.
2. The Provider stated (during an interview conducted on the date of inspection) that there were no updates to the plan.
Plan of Correction: A documented review of this plan shall be recorded.
Standard #: 22VAC40-111-90-C Description: Based on review, 1 of 3 children files indicated that Family Day Home did not obtain documentation of additional immunizations for a child (who was not exempt from the immunization requirements according to subsection B of this section) once every six months for children under the age of two years.
1. On the date of inspection (10/21/2019), the current immunization documentation on file for Child #2 was dated - 03/08/2019.
2. Based on information on file, on the date of inspection, Child #2 was under the age of two years.
Plan of Correction: The Parent of Child #2 informed the Provider that the next check up this child will be conducted 11/11/2019.
