Comments:
An unannounced renewal inspection was conducted today between the hours of 1:30pm through 4:30pm with the provider on site. There were 9 children (22 points) with the provider and an assistant caregiver. A sample of 5 children's files and 5 caregiver/household member's files were reviewed. The children were engaged in the following: naptime, handwashing, eating snack, free play, identifying objects and colors, singing nursery rhymes, and storytime. The physical space, emergency supplies, evacuation drills, attendance records, and the Emergency Preparedness Plan were reviewed. Areas of non-compliance are identified in the violation notice. Violations:
Standard #: 22VAC40-111-760-A-1
Description: Based on observation, the first aid kit did not contain the minimum requirements.

Evidence:
The first aid kit did not contain a first aid instructional manual.

Plan of Correction: I will get an instruction manual for the first aid kit.

Standard #: 22VAC40-111-770
Description: Based on observation, a working portable battery operated weather band radio was not available to caregivers at all times.

Evidence:
Provider had a portable radio which can be operated by battery or cranking mechanism. The available battery-operated radio would not turn on with batteries provided.

Plan of Correction: I will change out the radio.

Standard #: 22VAC40-111-60-B
Description: Based on record review, 3 of 5 children's records did not contain all required information.
Evidence:
1. The record for Child #2 did not contain the following information: phone number for father's place of employment, address for mother's place of employment, names of persons other than the custodial parents who are authorized to pick up the child.
2. The record for Child #3 did not contain the following information: address for father's place of employment, phone number for mother's place of employment, names of persons other than the custodial parents who are authorized to pick up the child.
3. The record for Child #4 did not contain the following information: name, address, and telephone number of father's place of employment.

Plan of Correction: I will ask the parents for the information.

Standard #: 22VAC40-111-90-A
Description: Based on record review and interview, the provider did not obtain documentation that each child has been adequately immunized before attending the family day home.
Evidence:
1. The record for Child #1 (start date 10/14/19) did not contain documentation of immunizations. Provider stated she has requested the documentation from the parents.
2. Immunizations contained in the record for Child #2 were dated 7/23/19, after Child #2 began attending the family day home. The first day of attendance documented for Child #2 was 7/8/19.

Plan of Correction: I will ask the parents to bring their shot records when they come to enroll their child prior to starting.

Standard #: 22VAC40-111-180-A
Description: Based on record review and caregiver interview, the provider did not obtain for each caregiver and adult household member a current Report of Tuberculosis Screening form every two years.
Evidence:
1. The record for Caregiver #1 did not contain documentation of a current tuberculosis screening.
The most recent documented tuberculosis screening available for review was dated 9/12/17. Caregiver #1 stated she has not gotten an updated tuberculosis screening at time of inspection.

Plan of Correction: I will get a TB screen. I will try to do it tomorrow.
