Technical Assistance:
Reviewed Safe sleep practices.

A mandated Monitoring inspection was conducted today. There were 4 children present (10 points) with 1 caregiver. Children were observed napping in appropriate equipment, playing with stuffed animals, manipulative toys and having one-on-one time with provider. Reviewed 2 children's records, 2 caregiver files, 2 household member files, attendance, emergency evacuation plan and drills, and physical plant. Standard #: 22VAC40-111-530-F
Description: Based on observation, soft bedding was being used around infants while the crib is occupied.
Child#1 (3 months) was observed asleep in a crib with 2 additional soft blankets stacked in the corner.

Plan of Correction: This violation was corrected at the time of inspection.

Standard #: 22VAC40-111-590-A
Description: Based on observation, Infants were not being placed on their backs when sleeping or napping unless otherwise ordered by a written statement signed by the child's physician.
Child#1 (3 months) was observed asleep on their stomach. Provider stated she did not have a physician's statement for the child to sleep on their stomach.

Plan of Correction: This violation was corrected at the time of inspection.
