Comments:

An unannounced monitoring inspection was conducted between the hours of 11:55am through 1:30pm. There were a total of 6 children present ranging in age from 7 months to 4 years (16 points) with the Provider. A total of 4 children's files and 2 staff files were reviewed. The children were observed napping throughout the inspection. Positive interactions between the provider and children were observed during this inspection. A complete inspection of the programming space, children and staff records, evacuation drills, menu, attendance, and emergency procedures were completed today. No new staff have been hired since the last inspection. The provider advised there was no medication on site today. The provider is current in CPR/First Aid certification. There was an abundant supply of age appropriate materials and toys and the home was clean and organized. Please contact me with any questions or concerns at morgan.bryson1@dss.virginia.gov.

Description:	Based on review of 4 children's records, 1 child's record did not contain documentation of a physical examination within 30 days after first day of attendance. Evidence: 1. Child #1's first day of care at the family day home was 8/26/19 and did not have a physical examination on file.
Plan of Correction:	I spoke to the child's parent and she will be bringing a copy of the physical this evening.
Standard #:	22VAC40-111-80-A
Description:	Based on review of 4 children's records, 1 child's record did not contain documentation that verification of child's age and identity was verified within 7 days of the child's first day of attendance at the family day home. Evidence: 1. Child #1 (First Day of Attendance: 8/26/19) did not have documentation that proof of identity was verified on file.
Plan of Correction:	I spoke to the parents and will complete identity verification and documentation this evening.
Standard #:	22VAC40-111-90-A
Description:	Based on review of 4 children's records, 2 children's records did not contain complete documentation of immunizations. Evidence: 1. Child #1's (First Day of Attendance: 8/26/19) record did not contain documentation that the child was adequately immunized before entering care. 2. Child #2 is a child under the age of 2 and did not have documentation that updated immunizations were obtained within the last 6 months. Child #2's most recent immunizations on file were dated 1/4/19.
Plan of Correction:	I spoke to both children's parents and they will be bringing me copies of the immunizations this afternoon.
Standard #:	22VAC40-111-180-A
Description:	Based on review of 2 staff records, 1 staff record did not contain documentation of an updated tuberculosis screening or test within the last two years. Evidence: 1. Staff #2's most recent tuberculosis test on file was dated 6/21/17.
Plan of Correction:	I will have a copy of the TB test today.
