Comments:
An unannounced Complaint Inspection was conducted on October 1, 2019 in response to a complaint that was received by the Virginia Department of Social Services, Licensing Division. Interviews were conducted with the provider and elementary school staff relating to allegation concerning a child finding a 9mm bullet in the provider's van on the way to school. Based upon a preponderance of evidence, the information gathered during the investigation supports the allegation, and the complaint is determined to be valid. Please complete the columns for 'description of action to be taken' and 'date to be corrected' for each violation cited on the violation notice, and then return a signed and dated copy to the licensing office within 10 business days. Description: Based on interviews, the provider did not ensure that ammunition is stored in a locked container, compartment, or cabinet during the family day home's hours of operation. Evidence: Identified through interviews or the provider, and school staff that a child found a 9mm bullet on the floor of the provider's van, too the bullet to school, showed a few friends and then turned it over to the school teacher.
