Technical Assistance:
**Please ensure that all assistants have completed the assistant's form for emergency contact, hire date, and orientation documentation. **Please send me documentation of rabies vaccination as soon as possible.

Comments:
An Unannounced Renewal Inspection was conducted today from 11:50am to 12:58pm. There were 9 children (28 points) directly supervised by 3 caregivers. The home, 3 caregiver records, 4 children's records, 1 household member's records (cat), evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed napping or getting their diaper changed. There was an abundant supply of books, toys, and materials for the children. The home was clean and organized. Areas of non-compliance are identified in this report. If you have any questions regarding this inspection, please contact the Licensing Inspector. Violations:
Standard #: 22VAC40-111-120-B
Description: Based on review, the provider did not ensure that assistants's records contained the required information. Evidence: Reviewed 2 assistants records and found that they were both missing emergency contact information.

Plan of Correction: Will have staff complete the assistants forms today.

Standard #: 22VAC40-111-200-C
Description: Based on review, the provider did not have documentation of orientaiton training that shall be signed and dated by the provider and the assistant. Evidence: Reviewed 2 assistants records and found that neither had documentation of completing orientation training.

Plan of Correction: Will have staff fill out the assistant's form by the end of the day.

Standard #: 22VAC40-111-460-C
Description: Based on review, the provider did not ensure that cats are vaccinated for rabies. Evidence: Reviewed the provider's records and found that the cat was not vaccinated within the last 3 years for rabies.

Plan of Correction: Will get the cat vaccinated by the end of November, after the cat gets over their current illness.
