Discussion was held on the topics of current enrollment, current staffing, and children's files. Copies of the following forms were given to the Provider: Child's Record, Information for Parents form, Provisional of EP&RP form, Declaration of Liability form, and the Health forms.

The following inspection took place between the hours of 8 am through 10:30 am. Today, I met with the Provider on site at the Family Day Home (FDH) located in town, Virginia. At the start of this unannounced, non-mandated, monitoring inspection there was 1 child observed (4 points) with 1 caregiver. A total of 1 child?s file, 1 staff file, and 5 household member?s files were reviewed. The child was engaged in the following: toy play, infant feeding, music, song, comfort, and other organized activities. Standard #: 22VAC40-111-60-B Description: Based on review, 1 of 1 child's record indicated that not all of the required information/documentation was contained within each child's record.
Evidence -
1. On the date of inspection (10/22/2019), the following information/documentation was unavailable for review: the complete information of two emergency contacts, the policy name and number of the child's medical insurance, the Information for Parent form, the proof of identification, the emergency medical authorization for Child #1, the Provision of the Emergency Preparedness and Response Plan, and Liability Insurance Declaration form.
2. The start date for Child #1 was listed as 10/15/2019.Plan of Correction: All missing information and documentation shall be completed and placed on file.
Standard #: 22VAC40-111-90-A Description: Based on review, 1 of 1 child's file indicated that before a child may attend the Family Day Home (FDH), the provider did not obtain documentation that the child had been adequately immunized according to the requirements of ? 32.1-46 A of the Code of Virginia and applicable State Board of Health regulations.
Evidence -
1. On the date of inspection (10/22/2019), a current immunization page was not available for Child #1.
2. Child #1 was in care on the date of inspection.
3. The start date for Child #1 was listed as 10/15/2019.Plan of Correction: An updated immunization page shall be obtained and placed on file for Child #1.
Standard #: 22VAC40-111-140-B-3 Description: Based on review and interview, 1 of 1 staff file indicated that the current certification in cardiopulmonary resuscitation (CPR), as appropriate to the age of the children in care, has expired.
Evidence -
1. On the date of inspection (10/22/2019), the current certification in CPR expired on 08/10/2019.
2. The Provider stated (during an interview conducted on the date of inspection) that she has not taken a re-certification class since that date.Plan of Correction: Staff #1 shall obtain a current certification in CPR.
Standard #: 22VAC40-111-140-B-4 Description: Based on review and interview, 1 of 1 staff file indicated that the current certification in first aid has expired.
Evidence -
1. On the date of inspection (10/22/2019), the current certification in first aid expired on 08/10/2019.
2. The Provider stated (during an interview conducted on the date of inspection) that she has not taken a re-certification class since that date.Plan of Correction: Staff #1 shall obtain a current certification in first aid.
Standard #: 22VAC40-191-40-D-5-a Description: Based on review, 1 of 5 household members files indicated that a person living in the home of a licensed family day home provider did not obtain a child protective services central registry (CPS check) within 30 days of a person in the home becoming 14 years old.
Evidence -
1. On the date of inspection (10/22/2019), a CPS check was not available for review for household member #5.
2. Based on the information on file, household member #5 turned 14 years since the last inspection.Plan of Correction: A request for a CPS check shall be completed, notarized, and sent within the next 10 days. Proof that this request was sent shall be email to the Licensing Inspector. Once this finding is obtained, it shall be placed on file.
