The following inspection took place between the hours of 8 am through 10:30 am. Today, I met with the Provider on site at the Family Day Home (FDH) located in town, Virginia. At the start of this unannounced, non-mandated, monitoring inspection there was 1 child observed (4 points) with 1 caregiver. A total of 1 child?s file, 1 staff file, and 5 household member?s files were reviewed. The child was engaged in the following: toy play, infant feeding, music, song, comfort, and other organized activities. If you should have any future questions, please call or send an e-mail to charles.perkins@dss.virginia.gov. Thank you.
