Discussion was held on the topics of current enrollment, current staffing, and emergency drills. As a reminder, fire drills are to be conducted on a monthly basis, and shelter-in-place drills are to be conducted twice a year.
The following inspection took place between the hours of 3 pm through 4:30 pm. Today, I met with the Provider on site at the Family Day Home (FDH) located near Aldie, Virginia. At the start of this unannounced monitoring inspection there were a total of 6 children observed (5 points) with 1 caregiver. A total of 7 children?s files, 1 staff file, and 3 household member?s files were reviewed. The children were engaged in the following: pick up, drop off, snack, homework time, learning, and other organized activities. Standard #: 22VAC40-111-830-A Description: Based on interview, emergency evacuation procedures (such as a fire drill) was not practiced monthly with all caregivers and children in care during all shifts that children are in care.
1. The Provider stated (during an interview on the date of inspection) that a fire drill has not been conducted for this year.
2. The drill log was not posted.
Plan of Correction: Fire drills shall be conducted on a monthly basis and recorded.
Standard #: 22VAC40-111-830-B Description: Based on interview, emergency evacuation procedures (such as a fire drill) was not practiced monthly with all caregivers and children in care during all shifts that children are in care.

Standard #: 22VAC40-111-100-A Description: Based on review, 2 of 7 children files indicated that the Provider did not obtain documentation of a physical examination (by or under the direction of a physician) prior to a child's attendance or within 30 days after the first day of attendance.
1. On the date of inspection (10/22/2019), documentation of a physical examination for Child #6, and Child #7 were not available for review.
2. The start date for Child #6 was listed as - 08/31/2019. The start date for Child #7 was listed as - 09/09/2019.
Plan of Correction: Documentation of a physical shall be obtained for all applicable children and placed on file.
