Comments:
An unannounced monitoring inspection was conducted today. The provider was caring for 2 children, totaling 6 points. One additional child was expected to arrive later in the day. Children's records, the physical plant, caregiver:child interactions, equipment and materials, caregiver records, training, health records, children's nutrition, and emergency preparedness items were inspected today.
The provider reported that no children's medications were on site today.

The children were observed playing with toys, engaging in creative play, practicing item recognition and color recognition, and interacting with the provider. The children were to be served macaroni and cheese, spinach, and milk for lunch later today.

Violations:
Standard #: 22VAC40-111-940-C
Description: Based on observations, it was determined that the provider did not store all perishable foods to be served to children inside the refrigerator.
Evidence:
One parent supplies her child's food from home. The food is brought inside a thermal lunch box and kept on the counter. The Inspector observed yogurt and homemade baby food inside the lunch box. The provider did not move these perishable items from the lunch box to the refrigerator.

Plan of Correction: I will move any perishable foods to the refrigerator when the child comes each day.

Standard #: 22VAC40-111-110-A
Description: Based on a review of children's health records, it was determined that two immunization records were not fully completed on the current form approved by the Virginia Department of Health.
Evidence:
The immunization records for child A and child B do not include the physician's signature on the statement indicating that the child is adequately immunized for his/her age.

Plan of Correction: I will ask the parent to have the statement signed by the doctor.

Standard #: 22VAC40-111-60-B
Description: Based on a review of children's records,it was determined that two records do not contain all required information.
Evidence:
Child record A and B do not include the last name for each emergency contact person.

Plan of Correction: I will ask the parent for the information.
