Technical Assistance:
Technical assistance was provided regarding the first aid kit and the requirement of at least two triangular bandanges.

Comments:
An unannounced renewal inspection was conducted between the hours of 11:30am through 12:55pm. There were a total of 4 children present ranging in ages 7 months through 5 years (10 points) with the Provider. A total of 4 children's files, and the provider's file were reviewed. The children were engaged in the following: playing games, completing a Halloween drawing, and preparing for lunch. Lunch served today is chicken nuggets, apple sauce, green beans, whole wheat bread, and milk. Positive interactions between the provider and children were observed during this inspection. A complete inspection of the programming space, children and staff records, evacuation drills, menu, attendance, and emergency procedures were completed today. No new staff have been hired since the last inspection. The provider advised there was no medication on site today. The provider is current in CPR/First Aid certification. There was an abundant supply of age appropriate materials and toys. Please contact me with any questions or concerns at morgan.bryson1@dss.virginia.gov.

Description: Based on record review, documentation of emergency evacuation and shelter-in-place drills was not maintained and available for inspection.

Evidence:
1. Documentation of monthly evacuation drills and 2 shelter-in-place drills per year were not available for the timeframe of October 2018-October 2019. The provider stated she has completed drills as required, however, she could not locate the drill log.

Plan of Correction: I have already made a new drill log and moving forward I will make sure to keep record of the drills.

Standard #: 22VAC40-111-100-A
Description: Based on record review, 1 of 4 children's records reviewed did not contain documentation of a physical examination prior to the child's attendance or within 30 days after the first day of attendance.

Evidence:
1. Child #1's first day of attendance at the family day home was 7/22/19 and did not have a physical examination on file.

Plan of Correction: I will get the child's mom to provide the physical to me.

Standard #: 22VAC40-111-60-B
Description: Based on record review, 1 of 4 children's records did not contain all required information.

Evidence:
1. Child #2's record was missing 1 of 2 required emergency contacts to include name, address, and telephone number.
2. Child #2's record was missing documentation of an annual review of the emergency contact information. The last annual review and update was completed 5/17/17.

Plan of Correction: I will get the parent to provide required information today.
