Comments:
Thank you for your assistance during this unannounced monitoring inspection. This on-site visit was conducted on October 28, 2019 from 10:50 a.m. through 1:15 p.m. There were ten children in the care of the provider and one assistant for a total of twenty-three points. Ten children records and the records for the provider, assistant and two household members were reviewed. No medication is being administered at this time. Areas of non-compliance may be found on the violation notice.

Areas observed: free play, movement/music, story-time, hand washing/diapering and bathroom procedures, lunch preparation, lunch and nap. Interactions between the provider and children were observed as positive and the children seemed comfortable with their daily routine and schedule.

Violations:
Standard #: 22VAC40-111-570-A
Description: Based on observation, one care giver was present with ten children and points exceeding sixteen.

Evidence:
1. Upon arrival at approximately 10:50 a.m., ten children were in the care of Provider #1. Points totaled twenty-three.
2. At approximately 11:10 a.m., the Provider #2 arrived.
3. Provider #1 confirmed there were ten children in care totaling twenty-three points.

Plan of Correction: If the situation arises, I will tell parents they can not bring their children until another provider is present. I will not exceed 16 points with one provider.

Standard #: 22VAC40-111-690-D
Description: Based on observation the provider failed to clean the diapering surface before sanitizing.

Evidence:
1. Diaper change was observed.
2. The provider was observed not cleaning the diapering surface with soap and water.
3. The provider was observed sanitizing the diapering surface.

Plan of Correction: A soap and water solution was immediately put in a spray bottle. The diaper surface will be cleaned and sanitized after each use.
