Comments:
An unannounced monitoring inspection was conducted from 2:00 ? 3:30 pm with the provider and her assistant. There were 12 children in care, ranging from 3 months to four years of age, for a total of 31 points. The children were observed waking from nap, washing hands, eating snack and coloring. Snack served today was applesauce, goldfish crackers and milk. Four children?s files were reviewed. Three substitute caregiver/household member records were reviewed and in compliance. Required information was posted. The last emergency escape drill was conducted: 10/18/19. The provider is Medication Administration Training (MAT) certified but there were no medications to review. Description: Based on a review of four children's files, and an interview with the provider, the provider did not obtain documentation that each child has received a physical examination prior to the child's first attendance or within 30 days after attendance. Evidence: The file for child #3 and child #4 (date of first attendance: 8/26/19) did not have documentation of a physical examination.

Plan of Correction: Will obtain the missing documentation.
