Comments:
An unannounced renewal inspection was conducted today form 10:30 am to 1:45 pm. There were 12 children (32 points) with 3 caregivers. The family day home has an abundance child appropriate toys, books and supplies . The physical plant, attendance logs, fire drill logs, emergency plans, menu's, cribs, bottles, caregiver training logs and emergency supplies were inspected. There were no medications on site. Children were observed reading books, playing with floor toys and infants were observed during feeding and diaper changes. Areas of non compliance were cited on the Violation Notice. Standard #: 22VAC40-111-830-A
Description: Based on a review of the evacuation log, the provider failed to ensure that emergency evacuation procedures shall be practiced monthly with all caregivers and children in care.

Evidence:
The provider did not have documentation of a fire drill for April 2019.

Plan of Correction: I will conduct a fire drill each month and document it.
I will conduct a shelter in place drill two times per year and document it.

Standard #: 22VAC40-191-60-C-1
Description: Based on a review of staff records, the provider failed to obtain an original criminal history record report within 30 days of employment.

Evidence:
Staff #3's record does not have documentation of an original criminal history record report .

Plan of Correction: CPS check form was mailed 3/20/2019 and 10/24/2019. I will contact OBI for follow up.
