Technical Assistance:
The license cannot be issued until the provider shows the Inspector the following: Central Registry and TB Health Screening results for household member B.

Comments:
An announced inspection for initial licensure was conducted today. The provider is not yet caring for children. This inspection was completed based on a review of the physical plant, a review of emergency preparedness items, a review of available on site toys and materials, and a review of administrative records.

Violations:
Standard #: 22VAC40-111-760-A
Description: Based on a review of emergency supplies, it was determined that the provider does not have all required emergency supplies in the first aid kit.
Evidence:
The first aid kit does not include more than one triangular bandage.

Plan of Correction: I will get another triangular bandage.

Standard #: 22VAC40-111-770
Description: Based on a review of emergency supplies, it was determined that the provider does not have all required items.
Evidence:
The provider does not have a weather band radio.

Plan of Correction: I will buy the correct radio.

Standard #: 22VAC40-111-790
Description: Based on a review of posted telephone numbers, it was determined that the provider has not posted all required information.
Evidence:
The provider has not posted the contact information for the emergency back up child care provider.

Plan of Correction: I will post the paper today.

Standard #: 22VAC40-111-800-A
Description: Based on a review of the provider's written emergency preparedness plan, it was determined that this plan has not been completed correctly.
Evidence:
The provider has not included a different "method to alert" sound for each emergency scenario included in the emergency plan.

Plan of Correction: I will update the plan and correctly complete the method to alert sections.

Standard #: 22VAC40-111-240-A
Description: Based on observations, it was determined that one area of the home has not been maintained in a safe condition.
Evidence:
In the backyard play area, the Inspector observed the following hazards in areas accessible to the children: stacks of large wooden boards, a portable trampoline, two rusted chairs, and various tools/materials used for landscaping.

Plan of Correction: Before children enter care, all items will be moved to the corner of the yard and blocked off with a gate.

Standard #: 22VAC40-111-480-D
Description: Based on observations, it was determined that the provider was not using all materials in accordance with manufacturer's instructions.
Evidence:
The Inspector observed two portable high chair seats on the floor. These seats are intended to be used only when safely secured to a full size chair. The provider indicated that she intended to use them on the floor. In addition, these chairs did not include a safety strap to secure them to a full size chair.

Plan of Correction: Before children enter care, I will get rid of those chairs and buy high chairs to use.

Standard #: 22VAC40-111-510-A
Description: Based on observations, it was determined that the provider does not have all required supplies to provide each child with an individual rest mat.
Evidence:
The provider does not have any rest mats for the children.

Plan of Correction: I will buy at least one rest mat and the linens to get started.

Standard #: 22VAC40-111-690-E
Description: Based on observations, it was determined that the provider does not have all required equipment to dispose of soiled diapers properly.
Evidence:
The provider does not have a step-on trash can with a leakproof liner to use for disposal of soiled diapers.

Plan of Correction: I will get a step on trash can to be kept next to the diapering station.
