Technical Assistance:
Discussion on staff TB screenings. PPD placement testing is not required, however this is one method to screen for symptoms of TB. A TB risk assessment model form is available on the public website.

Comments:
An unannounced Renewal inspection was conducted this date. A total of 10 children (29pts) were present with Provider and 2 assistants. When the Inspector arrived, the children were seated for lunch. Staff were observed assisting with serving and supervising their eating. Hand washing and bathroom procedures were observed after lunch. The children transitioned to the carpet area where they sang songs before nap time. Staff were observed assisting children to their cots for nap time. Observation was made of the FDH which included areas not normally included in the programming space. No medications on site today. The programming area was decorated with paper plate scarecrows, construction paper fall trees and leaves completed by the children. Activities planned for Halloween were being prepared. One topical ointment on site and was found inaccessible to children in care. A total of 7 children's records, Provider and 3 assistant records and two household records were reviewed today and found complete. Previous violation from the March inspection was reviewed and corrective action was verified today. This inspection began at approximately 11:30am and ended at 1:25pm. Description: Based on observation of the crib room, 12 inches was not found between occupied cribs.
Evidence: Two cribs located against the wall were occupied and did not have 12 inches of space between them.

Plan of Correction: We will re-arrange the crib room and allow for 12 inches of space for the cribs.
