Comments:
The licensing inspector conducted an unannounced monitoring inspection on 10/23/2019 from 3:07pm to 5:35pm. The children were napping upon the inspector?s arrival and the children were later observed watching nursery rhyme videos, playing games, learning through play in centers and playing outside. Today, there were 9 children, totaling 16 points, present with one caregiver. The caregiver was observed interacting with the children. An interview was held with the caregiver throughout the inspection. Additionally, when appropriate the inspector interacted with children.

The provider does not administer medication.
The provider does not provide transportation.
The provider does not have a pet.

Five children?s records, and one caregiver record was reviewed during the inspection.

Last shelter-in-place drill: 03/19/2019

The violations from the previous inspections were checked for corrections. There were no repeat violations found during today?s inspection.

Standard #: 22VAC40-111-60-B
Description: Based on review, the provider did not ensure three of five children's records contained all of the required documentation.
Evidence:
1. The record of child #1 (start date: 8/5/19 ) did not contain written documentation of the child's medical insurance policy number and one emergency contact's address.
2. The record of child #3 (start date: 9/18/19 ) did not contain written documentation of the child's medical insurance policy number and one emergency contact's address.
3. The record of child #4 (start date: 9/18/19 ) did not contain written documentation of the child's medical insurance policy number and one emergency contact's address.
4. The record of child #5 (start date: 3/9/18 ) did not contain written documentation of the child's medical insurance policy number.

Plan of Correction: I will get the information from parents today.

Standard #: 22VAC40-111-90-A
Description: Based on review, the provider did not ensure that one of five children's records contained documentation that the child has been adequately immunized before the child's first day of attendance.
Evidence:
1. The record of child #4 (start date: 9/18/19) had documentation of immunization records dated 9/27/19.

Plan of Correction: In the future I will make sure to obtain immunizations before the children start.

Standard #: 22VAC40-111-680-B
Description: Based on observation, the provider did not ensure that children's hands were washed after diapering.
Evidence:
The provider did not wash child #1's hands after diapering.

Plan of Correction: In the future I will make sure the children's hands are washed immediately after mine. The child did not touch anything during diapering.
